Weight management scheme

What is the scheme and how does it work?

The weight management scheme is designed to initiate long term lifestyle changes to help you achieve and maintain a healthy weight.

 

The scheme is free of charge and encourages you to eat healthily and increase your physical activity levels.

 

You will be offered a series of personalised one to one appointments over six months with a weight management adviser in a local pharmacy. You will also attend an appointment with a dietitian, with the option of a further group dietitian session.

 


  

I am interested in the scheme, what do I do now?

To see if you are eligible to join the scheme (inclusion/exclusion criteria applies):

 

  • self refer by calling 0800 849 4000

  • contact your local pharmacy (please be aware that not all pharmacies offer this scheme)

  • ask your GP, practice nurse or dietitian.
