Enhanced service for the provision of medication administration record (MAR) charts – service level agreement
The overall aim will be to provide medication administration record charts, for those patients assessed by the Medicines Support Service as requiring formal carers to administer medication. In particular, the enhanced service will:

Ensure that the carers are aware of medication changes.

Provide the carers with pharmaceutical advice as required.

Assist in medicines management and reduce waste.
-
Ensure a full record of each patient’s medication is stored both at the dispensing pharmacy and on the patient’s GP system.
Enhanced service for the provision of monitored dosage systems (MDS) – service level agreement
The overall aim will be to provide monitored dosage systems (MDS) for those patients assessed by the Medicines Support Service as being unable to self administer their medication by other means. In particular, the enhanced service will:

Ensure that the ongoing provision of the MDS is appropriate.

Monitor the patient’s ongoing concordance with medication
-
Assist in medicines management and reduce waste.

Ensure a full record of each patient’s medication is stored both at the dispensing pharmacy and on the patient’s GP system.

Palliative Care and Urgent Care Medicines
Local enhanced service for supply of palliative care and urgent care medicines services by community pharmacists – normal working hours 2010/11
The aim of this service will be to provide an integrated Palliative Care and Urgent Care Medicines service across primary and secondary care in Eastern & Coastal Kent PCT. The Enhanced Service will be to:
- Improve and ensure the availability of palliative care and urgent care medicines from community pharmacies during normal opening hours.
- Provide community pharmacists and healthcare professionals with contact details of pharmacies in their locality who have committed to stock the full range of palliative care and urgent care medication as per the Eastern and Coastal Kent PCT Palliative Care and Urgent Care Formulary.
- Support and provide up to date information and advice on prescription writing and dispensing in order to reduce the number of difficulties experienced by carers and relatives in obtaining supplies.

Anticoag Dispensing and Anticoag Monitoring Services
Enhanced service for monitoring anti-coagulation medication - service specification
The overall aim will be to provide an integrated anticoagulation service across primary and secondary care. In particular, the Enhanced Service will:

Provide more services that are near to patients and are easily accessible
-
Ensure that maintenance of patients is properly controlled and the need for continuation of therapy is reviewed regularly and therapy is discontinued where appropriate.

To improve safety, access and convenience for patients who receive anticoagulation therapy.
-
To take into consideration recommendations from the NPSA report on the safety of anticoagulation therapy services.
-
To assist in medicines management and reduce waste.
-
To ensure a full record of each patient’s medication is stored both at the dispensing pharmacy and on the patient’s GP system.

To refer patients to specialist services where appropriate.
