• To work with secondary care to enable a more rapid and productive journey for dementia patients.

• Listen to patients, carers, relatives and GPs in order to implement and measure improvements
• Improve communications between GPs and consultants.

How you can be involved:

• Use existing forums, eg GP meetings
• Out-of-hours organisations
• Dementia/alzheimer cafes for communication.
