Legacy documents

National Quality Board guidance identifies the need for “a robust system of handover that effectively captures and transfers organisational memory” for
use during NHS transition.


The guidance requires each outgoing NHS organisation to produce a transition document.

The Kent and Medway Cluster document will include:

 

  • information on health services provided to the local population;
  • a ‘Pen Portrait’ of the Kent and Medway with facts and figures on population and geographical boundaries;
  • current position on quality, finance, performance, capacity, and people;
  • relevant organisational memory – historic financial positions, recent reconfigurations, long-standing issues;
  • future challenges/risks ‐ the cluster risk register will be included in due course to capture each of the above issues with proposed mitigating actions;
  • a library of knowledge/skills – a depository of all useful resources such as strategy documents, consultancy reports; and
  • a directory of services and skills available
